Background

Established By: Thomas G. Slama, M.D., FACP, to help students who plan to study medical science and who need financial assistance. Dr. Slama wanted to do something for the local community and schools in recognition of the benefits enjoyed by him and his family.

Residency: Boone County, Indiana
Diploma: Zionsville Community High School
Enrollment: Any accredited* public or private college or university
*(Accredited by the North Central Association of Colleges and Schools.)
Specialized Study: Medical Science
Special Characteristics: No (involvement in extracurricular activities, community, church and employment impact evaluation between candidates)
Special Preference: No
Financial Need: Yes (family income level impacts evaluation between candidates). Provide a completed College Cost Estimator and Boone County release form (PDF 388K) in a sealed envelope and return it to the guidance department.
Minimum GPA: No (but GPA impacts evaluation between candidates)

  1. Community Foundation Scholarship Application Form (PDF 128K) [Note: this application form can be filled out on-screen using the Adobe Acrobat Reader software and then printed.]
  2. High School transcript including SAT/ACT scores
  3. Two reference forms (one may be from a family member) Must address perceived ability of student to succeed in medical science course (Community Foundation Scholarship Recommendation Form (PDF 52K) [Note: this application form can be filled out on-screen using the Adobe Acrobat Reader software and then printed.])
  4. Essay
  5. College Cost Estimator and Boone County release form (PDF 388K) in sealed envelope.

Note: Incomplete applications will not be considered.
Essay to Include:
  • Description of yourself.
  • Motivation for your decision to pursue medical science studies and any past activities or achievements, which may have motivated these decisions.
  • Future aspirations.
  • Format: Typed, double-spaced
  • Minimum Length: Two pages (excluding cover)
  • Maximum Length: Five pages (excluding cover)
  • Cover page with title, name, address, and phone
  • Note: Essay will be evaluated for content, style, grammar, punctuation, spelling, etc.
Selection By: Slama scholarship selection committee; approved by the Board of Trustees of the Community Foundation of Boone County
